This Nevada Maintenance Independent Contractor Agreement is made and entered into as of this [Date] day of [Month], [Year], by and between:

Independent Contractor Status:

The parties agree that the Contractor is an independent contractor and not an employee of the Client. Nothing in this Agreement shall be construed to create an employment relationship under Nevada Revised Statutes NRS 608. The Contractor is solely responsible for all federal and state taxes, including self-employment tax, unemployment insurance, workers' compensation, and any benefits. The Contractor is responsible for all business licenses and compliance.

Insurance:
